1. General Provisions

1.1. These Terms of Service define the rules for using the website available at easf.eu (hereinafter referred to as the "Service").

1.2. The Service enables joint completion of the European accident statement form by two parties involved in an accident.

1.3. Use of the Service is voluntary.

2. Definitions

  • User – a person using the Service to fill in the accident statement form.
  • Session – a joint working session created by two users to fill in the form.
  • Form – an electronic form of the joint accident statement compliant with European standards.
  • PDF Document – a generated and emailed document containing the completed form.

3. Terms of Use

3.1. Using the Service requires:

  • A device with Internet access and a web browser
  • An active Internet connection
  • Acceptance of these Terms of Service and Privacy Policy

3.2. The User undertakes to:

  • Provide truthful and accurate information
  • Use the Service in accordance with the law and good practices
  • Not engage in activities that may disrupt the functioning of the Service
  • Not share session access data with third parties (except the other accident participant)

4. Form Completion Process

4.1. The User creates a new session that generates a unique identifier and QR code.

4.2. The other accident participant joins the session by scanning the QR code or opening the link.

4.3. Both users fill in the form in real-time, each on their own device.

4.4. After completing all required fields, users provide electronic signatures.

4.5. After signing and providing consent, the generated PDF document is sent to the specified email addresses.

5. Liability

5.1. The Service Administrator is not responsible for:

  • The accuracy of data entered by users
  • Legal consequences arising from the content of the completed form
  • Acceptance of the document by insurers or other institutions
  • Service interruptions due to force majeure or third-party actions
  • Data loss caused by technical problems on the user's side

5.2. The User bears full responsibility for the content of the data they enter and for the legal consequences of providing an electronic signature.

6. Copyright

6.1. All rights to the Service, including copyrights to the graphic layout, source code, and content, belong to the Administrator.

6.2. Copying, modifying, or distributing elements of the Service without the Administrator's consent is prohibited.

7. Complaints

7.1. Complaints regarding the functioning of the Service should be submitted through the contact form available on the main page.

7.2. A complaint should include a description of the problem and contact details enabling a response.

8. Changes to Terms

8.1. The Administrator reserves the right to make changes to these Terms of Service.

8.2. Users will be informed of significant changes through a message on the Service's main page.

8.3. Using the Service after changes are introduced means acceptance of the new Terms of Service.

9. Final Provisions

9.1. For matters not regulated by these Terms of Service, Polish law applies, particularly the Civil Code.

9.2. The Terms of Service come into effect on the date of publication on the Service's website.
